fix The Forging of Quel'Serrar
...
because the Unfired Ancient Blade can't be used
the Treated Ancient Blade is now added to Onyxia's loot table.
it will only drop if a player has the quest The Forging of Quel'Serrar
- corrected flags for related quests and items
- removed the spell from the Unfired Ancient Blade
- fixed skinning loot for Onyxian Warder

fix(DB): Added an lfgdungeons_dbc entry for 40-man Onyxia
...
This fixes an issue where scaling was not working for 40-man Onyxia with a recent AutoBalance update/rewrite which checks for acceptable level scaling ranges from LFG dungeon data.

This fixes a minor issue where 40-man Onyxia was not identified as a dungeon/raid boss by `Creature->IsDungeonBoss()`
